If you want to enter the large crack in the wall, you have to kill Mom in 20 minutes or less. At which point Mom's Heart will be replaced by It Lives, the Cathedral/Sheol: Kill Mom's Heart 10 times. A beam of light and stairway will appear after Beating It Lives.
